What hypothyroidism does to weight — and why "normal" labs don't always mean normal metabolism.
Overt hypothyroidism
A clearly underactive thyroid (elevated TSH, low free T4) is associated with significant weight gain and a meaningful reduction in resting metabolic rate. Even with treatment, many patients don’t return to their pre-illness weight — partly because the weight gain triggers additional metabolic changes that persist even after thyroid levels normalize.
Treated hypothyroidism
Patients on thyroid hormone replacement who are biochemically euthyroid (normal TSH) may still have persistent metabolic changes — lower resting metabolic rate, altered fat distribution, impaired thermogenesis — that make weight loss harder than it would be in a patient without thyroid history. Medication normalizes the hormone. It doesn’t always fully normalize the metabolism.
